Change Complexity Questionnaire

One of FIVE questionnaires in the "Change Dashboard"

You are invited to complete a Change Complexity questionnaire. This is designed to determine complexity of the intended change/project.

A profile will be produced, based on all completed questionnaires. It will form part of a Change ‘Dashboard’ that will show the organisation’s overall capability across a number of criteria. If you have any problems in completing this questionnaire please contact us.

Please allocate a score (out of ‘7’) for each question in the light of the situation. This might be in the context of your: organisation (as a whole), your own department, your work team, a project you are on, or a change programme.

PLEASE NOTE:

If you exit the questionnaire before completing the questionnaire and pressing the “SUBMIT” button you will lose the scores that you entered.

Be careful when using the scroll wheel on your mouse. The scroll wheel will change the ‘score’ if the box is highlighted.

Filling in the scores

1. Click on Question 1 score button to see the dropdown list
2. Click on your desired score
3. Now press the “Tab” key
You will now be on Question 2
4. Using the numbers on your keyboard type your score
5. Now repeat steps 3-4
6. To finish click on the “Submit” button mouse.


In considering your answers please use the (number) range scale as follows:
7Exceptional level of truth
Statement is invariably (almost 100%).
3Reasonable level of truth
Statement is on occasions (about 25%).
6Very high level of truth
Statement nearly always (almost 90%).
2Minimal level of truth
Statement is sometimes (about 10%).
5Good level of truth
Statement is generally (about 75%).
1Negligible level of truth
Statement is rarely (almost 0%).
4Satisfactory level of truth
Statement is often (about 50%).
